We accept most dental insurances. For our patients without dental insurance we do provide a discount. We look forward to serving you in the near future.
After you have scheduled your first appointment with Serene Family Dentistry, please take a moment and complete the following required medical forms. You can complete these forms when you come to our office, but it may be easier for you to do this now and allow us to see you more quickly when you get to our office!

We will file your insurance forms on your behalf. And for patients who do not have dental insurance we offer a dental discount program. Please contact us for more information on accepted insurance plans or discount programs available.
