Patients Information
At Serene Family Dentistry, we accept nearly all dental insurance plans. We also offer discounts to our patients who do not have dental insurance. We don't want to see finances get in the way of your oral health.
Once you schedule your first appointment with Serene Family Dentistry, please complete the required medical forms below. If you would rather do them in person when you come to our office, you can, but it may be quicker and easier for you to fill out the forms now! That way you can see the dentist right away instead of being slowed down by filling out paperwork when you arrive for your appointment.
